I have read awe's Privacy Policy and agree that Woolworths Limited and awe may exchange such private information as awe and Woolworths Limited consider necessary for awe to confirm my eligibility for membership of awe, to maintain my membership records up to date, and update my contact information held by Woolworths Limited.
I have read the Terms & Conditions of awe and agree to abide by these Terms and Conditions.
I apply for membership in the association of woolworths employees (awe).
I authorise my employer to deduct from my pay such amount as may from time to time be determined and required from my membership dues in awe, subject to my right to revoke this order at anytime by written notice to my payroll clerk.
I acknowledge as a monthly paid employee this application will be forwarded to Corporate Payroll and deductions will commence from my next pay cycle. A copy will be sent to Corporate Payroll to be kept in my file. I acknowledge as a fortnightly paid employee this application will be forwarded to NPO and deductions will commence from my next pay cycle. A copy will be sent to NPO to be kept in my file. I acknowledge as an EBA employee I am required to print my emailed application form sign and give it to my Office cashier. This copy will be kept in my file.
